CDM Non notifiable form

Hi all

I'm looking at putting a form together for our Estates team to complete in planning of works for non notifiable projects to ensure they are meeting all their responsibilities. We use an external company to manage all of our CDM notifiable jobs but this is for all the small jobs that don't need an F10 but still have duties to complete from the Estates team.

Can anyone help?
