Ireland_'Hopes fading' over missing Coast Guard helicopter crew
"One crewman has been located in the search for four crew of an Irish Coast Guard air and sea rescue helicopter which is missing off the Co Mayo coast.
It is understood the crewman, who is in a critical condition, was spotted at first light by the Shannon-based Sikorsky S-92 helicopter and was picked up by the Achill-based RNLI lifeboat.
Wreckage has been located in the search area between 8km and 10km off Blacksod lighthouse in north Mayo.
The Dublin-based helicopter went missing off the Mayo coast shortly after 1am on Tuesday."

"A preliminary report into the fatal crash of Coast Guard Rescue Helicopter 116 has found that an onboard warning system on the aircraft did not include data relating to Blackrock Island.
The helicopter crashed into the island off the coast of Co Mayo on 14 March, with the loss of all four crew members on board."

I have been trying to read some of the technical things from a non-technical knowledge to try and note what specifics caused this terrible disaster and all I can deduce from it is that, perhaps, owing to the weather restricting clear views, they possible made the decision to fly lower thus should be able to physical see what was ahead as the actual Blackrock Island was not included in their data.
If this was the case, then one part of the technical things on board would then be "inhibited", so would not send a warning and instead would leave it up to visual adjustment only.
So very sad at the thought of their last moments.
I do hope they find the missing crew members.

"Aviation authority asks pilots to help correct charts
Regulator admits lighthouses mislocated and symbols obscured on maps
The Irish Aviation Authority has asked search-and-rescue and other pilots to help correct the aeronautical charts they use. Lighthouses in the wrong locations, missing spot heights and obscured symbols are among the errors in the authority’s latest visual-flight-rules 1:500,000 chart, published last June, it has conceded."

I have read all of this (with the assistance of Google on abbreviations) and I am absolutely horrified as to what has been suggested in the report against the operator's safety factors.
Four lives taken that could have been avoided.

I was first taken aback by an Executive Summary which stretched to 21 page.
Few Executive Summaries ever get anywhere near that.
When I started reading it realised what a shambles their entire system is.
The actual report is 350 pages.
For comparison, the Clutha Police Helicopter Crash Report was 176.
Words fail me.
One thing that immediately struck me was that the Irish helicopter cockpit lighting had been modified to be compatible with Night Vision Goggles, but Goggles were not being used. This, the crews felt, made the lighting poor.
This was something we found on RAF Tornado NVG lighting modifications in Gulf War One in 1991, it was sorted out in a matter of weeks.
How can the Aviation Industry be having the same issues almost 30 years on...
