I continue with the RRC IGC1 textbook. My doubts today;
Analyse info;
When a witness tells what happened, what is better?
a) Investigator takes notes, and when witness ends story then investigator reads notes to witness, and if witness disagree the investigator change what needed.
b) investigator asks witness to right and sign a statement telling what happened.
Reasons of accident; Immediate + underlying + root causes
Underlying and root causes are not the same? What is exactly the difference?
Indentify suitable control measures;
- prevent same accident, same place?
- prevent similar accident, similar place?
If the answer to both is no, it's the wrong measure. But what if the answer is no to one of the questions, and yes to the other? then it'd be ok?
