Health and safety audit scoring matrix

Hi all,

I have a general H&S audit template which covers a number of questions under 4 headed sections, what I am looking to do is to create a scoring matrix for each section or question. I'm struggling as to what would be the best to use as this would need to be easily understandable to the managers of the area being audited.

Has anyone got anything I could use or suggest/recommend anything?
The light at the end of the tunnel has been turned off due to budget cuts..............

I have a spreadsheet audit which has 23 subjects (a page for each) from safety meetings to legionella etc. which is all linked to results, a graph and somewhere for the findings and photos. Anyone want a copy just pm me.

There isn't a correct answer to be honest, mine is a simple 1 - 3 score:

Good = Standards are met 3
Improvements = Improvements necessary 2
Urgent = Remedial action required 1

Topics I am covering are PPE, Behavioural, Housekeeping, Machinery Training (H&S)

This will be like a score card by Team and made available to Managers, TLs and Supervisors, hopefully this will push H&S forward as no one wants a low score, not when the matrix is going to Senior Management.....
