Re: H&S Annual report
I would say it depends entirely on the recipient(s) of the report.
Is this a private or family owned company, or does it have shareholders?
Will it be publicly available or just for a limited distribution?
Are you thinking of a page or a half dozen pages?
Will it be stand-alone document or included in a brochure/report of some sort?
All these sort of factors influence how you compile it.

Re: H&S Annual report
As Berni says, depending on the purpose of the report and its intended audience you could always look at the output of the Management Review if the business is 18001/9001/14001 certified as these have a structured approach to what needs to be in a Management Review.
So when the whole world is safe..............what are we going to do then?

Re: H&S Annual report
The most obvious things to report on are :-
You could also collate costs showing that the protective measures cost less than the cost of absense and injury, the cost of defending claims, the potential cost of fines.
For a medium size business I would steer clear of accident rates per 100000 hours etc as these are statistically irrelevant in anything but a huge company or a national survey.
You want a report that states what the situation is, compared to previous years and setting targets for the future with a little bit about how you plan to achieve it.
- Accidents, how many in the year,
how long were people off work sick or injured,
what hazards and risks were identified
what corrective actions have been taken
how are they working (eg do the employees wear hard hats where they didn't before, do they wear gloves more than they used to.)

If it isn't broken, that doesn't mean you can't improve it. (Do three negatives make a positive ?)

Re: H&S Annual report
I just completed a brief 9 page annual report to a small construction company
As already mentioned, the amount/detail depends on the company size/type and management requirements
