Does anyone have an tips or experience of last minute RA's systems, can advise on a good one, or some do's and don't
need to implement a procedure that all staff stop and think be fore they undertake a task, and not proceed with task if not safe to/ don't have the correct info to do so.
a few near misses have prompted this need.
from your experience do they need to be recorded or just verbal interaction.

Update: taken a new job hopefully confidence and positivity will return with new challenge - scary though leaving current place after 16 years (6 years H&S !)

Depends on the potential dangers but this is where the "dynamic" risk assessment and Human Factors comes in. The people who use this most effectively are our emergency services - police chasing criminals up a back alley or firefighters going into a blazing building.
But to make it work - they need good risk assessment / hazard awareness training - excellent communications with leaders - and a robust management system of procedures that have looked at all known - and many "potential" - hazards and risks.
Personal discipline - continuous practiced and rehearsed scenarios - are the only way to ensure that the reactions of people are within laid out parameters. Followed by detailed debriefing and analysis of the results - implementing any learnings followed with more training on the findings.
I worked with a major Scottish Police force to make them aware of the "Red mist" danger as they called it and it they used some examples of where it had went badly wrong to reinforce the message.
This may be much more than you require as your people can maybe able to walk away - but I would suggest the strategy would be effective.

The Council I worked in used a "take 5" risk assessment process for their road working teams as part of a dynamic risk assessment piece. There's loads of examples online - give it a google and see if there's something that fits...

I have used IAuditor for many things, you can even customise their template to suit your own needs.
I have used it for auditing, rolled it out to the gangs for onsite RA's where there was a requirement and given training on it use, however going back to the original point, all the staff using this or requiring to use it for RA use will need more than smartphone/tablet training as they will also need some level of risk/hazard awareness training.
IMO there is no point rolling out a system that although 'SMART' the users are not competent to use.
