Accident Investigation training DVD

Hi. I am looking for an up-to-date, simple Accident Investigation DVD to show as part of a refresher H&S training day focusing on basic accident investigation (NB don't want too much detail and really need to keep it simple). I have looked at the ones by Safety Media and they are too detailed for what I need and/or out of date (huge computers, ancient camera equipment!). Any suggestions? Thanks!
