Janie is dead right, the law says record if you employ 5 or you have to record the findings, nothing about having it signed.

I would say, (with refernce to HSG65 pp17) that the four Cs are what you are looking for:
control (within the organisation)
co-operation (between individuals)
communication (throughout the organisation)
competence (of individuals)
