Hi all I am looking for a good template to use to carry out an assessment and wondered if anybody had one that felt was effective and also better than the one off the HSE site?

I admire your commitment but I have been there and done that and its not effective safety management and we have to look at it this way, if there was an accident there when you wasn't. Photos is great but you could just leave his blank but just agree, smile and let him deal with the questions of why he is the only one without a photo and most will know him anyway. Bigger battles to fight in my opinion.
