We have a couple of critical safety procedures that we have a one strike and you are out. In fact the reversing of a refuse truck without an assistant is so strict that if caught all the crew get sacked. We lost a lot of people and some got panicky due to losing drivers when they are like gold dust....

I see a near miss/hazard as spotting something at the time it is seen by any member of the workforce, our safety observations are snapshots of a process watched by a supervisor noting the good and the bad.

They should be clearly visible but was always told don't need to be in every room. I wouldn't avoid having toetectors, reasonably practicable?
Depending on size of warehouse you may also want to put in a distance rule where if an flt driver sees a pedestrian within a certain distance they must stop and acknowledge each other

As said above the risk assessment will play a big factor but if you your warehouse guys wear PPE then so should pedestrians unless you have physical segregation in place. What if the stock they are checking falls on their toes? Had this issue before and some office staff didn't want to put boots on ...
