I will second IAuditor, I created a form specifically for an audit on our remote sites, the intention was to create a system that was less open to interpretation issues.

It worked a treat, it's very powerful and you can a signature at the end.

I looked into this when they were welding on one of my sites, They were using shielding but there were places that were exposed.
