Hello Brian,
You have already answered your question! As for the actions taken, it would very much depend on the circumstances of each case. The HSE publish a very good and free Stress RA. Just go to the main site and type stress in the serch bar.

oooh I am all up to date with the CPD and feeling good about it!. Once you understand how to fill out the required fields it's quite easy.
