I did a very good accident investigation course in Cranfield university. Not cheap but very well run. They also have different levels.

footing a step ladder... I don't think I would insist on that to be honest. I would ask that they are used correctly but not footing it.

I would not consider this RIDDOR reportable unless there is an issue with the building infrastructure that might have contributed to the injury.
