wonder if I can pick your brains. I have been asked to develop / update a health & safety audit sheet which is completed by managers and supervisors on site.
What is required is a 2 page sheet with one side consisting of Health related issues and the other safety related.
at the moment we have a sheet that has about 20 questions and the person completing is asked to select 10 of those questions and rate the area between 1 -10, the total score at the end equals there overall percentage.
I just wonder if any one has any thing similar, or how you would go about updating or creating new. I wonder if the scoring system is needed or if the areas should be broken up while trying to keep to a page each and having every thing answered rather than say 10 out of 20. as I find most people just go for the most common 10 making it more a tick box exercise rather than an audit tool.

Would I be right in your OP that at the moment the audit system seems to be more of a tick the box than a valid or worthwhile exercise. If so, before creating another document to be ignored, i would look at why the current system is failing so that any new one has a greater chance of success.
If our managers can choose which ones to do, probably the easiest / quickest to complete, then morph it round so you/someone chooses the topics they must look at, such as ones that are currently failing so they have to complete 5 essential and 5 self chosen. This allows you to change the 'essentials' to suit the failings.
On the face of it your system seems sound and perhaps its more in its application. What happens to the completed forms, if they are not monitored, actioned or feedback given, then we can understand why they are not being taken seriously by the managers. If, in truth, you are checking and filing them, perhaps its time to review the whole process. Before a whole host of new forms that may create more divide, I might look at engaging with the managers to boost the current process.
