my answers are
1) The person has become complacent
2) The person hasn’t been made aware of the seriousness of the hazard
3) The person is ignorant.
4) ???
im pretty sure the first 2 are ok, 3 is a bit of a stab in the dark, and i cant think of another? any help would be gratefully received!
No 4 could perhaps be fatigue either work related (not had a break), or even because they have problems at home, had a night out or similar, thus not enough sleep (new baby).
Other reasons may be lack of correct safety awareness training, perhaps the machinery being used isn't working correctly and instead of calling maintenance, one becomes edgy and frustrated to get the job done, so impatience and cutting corners causes injury, housekeeping issues like messy or oily floors causing slips, area not sufficient in size to work freely and safely.
