Voluntary H&S Survey Questionnaires
Dear participant,
You have been requested to participate in a confidential dissertation research study conducted by myself, Ning Law from the Bartlett School of Construction and Project Management at University College London. This piece of research being carried out at part of my studies leading to the award of BSc (Hons) in Project Management for Construction. The aim of this study is to investigate the effect and implications of the new 2016 Sentencing Guidelines on the future management of health and safety within the construction industry.
As a construction industry professional, your opinion will be very valuable for me and therefore you have been requested to complete as short confidential questionnaire/structured interview questions designed to obtain information primarily around how you think the new sentencing guideline will influence working in practices with regards to health and safety in construction. The expected time for the completion of this questionnaire should not take longer than approximately 15 minutes.
Your participation in this dissertation study is voluntary. You may refuse to take part in this confidential research or decline to answer any particular question you do not wish to answer. However please note that the information provided by you in this set of questionnaire/structure interview questions will be used only for this dissertation research purposes. The information that you give me will be kept strictly confidential so as to prevent any exposure of identification from individual respondents, therefore can I ask you not to put your name or any other company information as part of your response to this questionnaire.

Can I thank you most sincerely for your comments which will not only make a valuable contribution to my studies, but will also help me to better understand how the construction industry is responding to these new 2016 Sentencing Guidelines.

Filled and submitted (using a fake email address) - don't tend to hand this out in fear of getting spammed.
