looking to put together a training session and presentation/ info sheet for staff in march on Root cause analysis to improve our use of this in incident investigation/ reporting.
will be delivered to on the ground staff, so needs to be the right tone.
any tips? good websites ?

Update: taken a new job hopefully confidence and positivity will return with new challenge - scary though leaving current place after 16 years (6 years H&S !)

I can't recommend any particular website but Mr G does offer a lot of them when searched

I have found the simplest method of training root cause analysis is to use 5 why. Once they have that in their mind you can then move to more complex and more detailed methods. To be fair though, for basic cause analysis I don't think you can beat 5 why.

I find the why tree confuses non safety people and we ended up phasing it out and replacing it with what we call a "Why4safety" it's a simple and faster process in my opinion and our root causes on investigations have improved beyond our expectations and all it is is a spidegrame but the form has a blank box in the middle for the incident and around it a box with an area to cover such as Plant/Machinery, Environment, people, Training etc. They are encouraged to do this on a white board as a group or with post it notes and the feedback from everyone was brilliant and because it is so simple and quick they do it for everything. Before none LTIs were poor investigations as they felt that the LTI's got the most scrutiny.
For brilliance and, at the same time, for consistency of achievement, for activity in philanthropic enterprise, for astuteness of management and for general alertness, the superiors of Aston Villa cannot be found
