I am trying to source a PTW Audit Form or template that I could use.
Had a look on various websites but not much luck. Could anybody drop me a line or attach me one please.

This post has made me think that we do not have a formal audit process for checking hot work permits are audited regularly.
The current audit process is currently very ad hoc and relies on when FM staff (or me) having the odd spare 15 minutes, or if I pass by somebody engaged in a hot works activity (our HWP system also covers dusty work that make require a fire alarm isolation). As a result, very few audits are undertaken and no records are kept.
Apologies for slightly hijacking the thread, but can anyone give me any examples of a hot work permit system in terms of how often? Who carries out this audit? and what penalties are given where a contractor or staff member if they are not complying with conditions of the permit?

Messy,
I do a lot of work with an offshore drilling contractor who audit as a minimum, 10% of hot and cold permits and 20% of entry permits.
Audit can be conducted by anyone on board at the time.
