Don't know if it's the sort of thing you are after but I have a safety alert about a loose fitting harness where the wearer fell and both testicles popped out of the sack. This does contain graphic pictures and descriptions. pm your e-mail and I'll forward it if you like.

I don't know where your information is coming from but the current TWA 8 hr exposure limit for hydrogen sulphide is 5ppm. 10ppm is for short term exposure (15 min)

I think I can see where Dave is coming from. If during cleaning operations you are turning the isolator to the off position how much more effort would it take to apply a caliper and padlock to ensure that no-one can accidently re-energise the conveyor. May seem like belts and braces but it's better ...
